PHYSICIAN ORDERS FOR LIFE SUSTAINING TREATMENT (POLST)
POLST is a Physician’s Order that outlines a plan of care reflecting a patient’s wishes for end of life care. It is designed to improve the quality of care individuals receive at the end of life by ensuring that a patient’s wishes, including life-sustaining medical interventions and care, are honored by health care providers. The POLST form does not replace the Advance Health Care Directive.
